Before your player can take the ice with the Tomahawks, you will need to fill out the following forms and mail them to Amy LaPoe. Once your applications is accepted you will be given directions on how to register online with USA Hockey and AHAI to complete the Tomahawks registration process.

In addition to the registration applications, an emergency form and insurance form will need to be completed for each participant. Regardless of the athlete’s age, we require the parents/guardians to sign the forms. Athletes with Down Syndrome need to submit a letter from their doctor that indicates the athlete does not have Atlanto-Axial Dislocation, along with the date of their X-ray.

United Center Instructions

Dear Tomahawk Parents, Players & Coaches:

PLEASE READ THE FOLLOWING INFORMATION CAREFULLY as it should answer any questions you and your guests might have regarding your game day.

Season Ticket Holders: Parents and team members who have purchased Season Tickets or already have tickets to the Blackhawks game on the night of their TomaHawks game do NOT have to purchase extra tickets to be admitted into the United Center for the TomaHawks game. They will need to enter through gate 6 for the TomaHawks game.

  • Please plan accordingly for games that have an early start. Tomahawks games will not be postponed due to heavy traffic or late arrivals.
  • Blackhawks game:      6:00 pm

Tomahawks game:     2:40 pm

Arrive at loading dock:            1:55 pm

  • Only Tomahawk Coaches will be allowed in the locker rooms and on the benches during the game. No parents will be allowed in the locker room area.  For any special circumstances contact Amy or Melanie.  Special circumstance examples – Your Tomahawk is very young or needs a parent to calm or communicate for them.
  • Equipment / players / coaches must be dropped off and will enter the United Center through the loading dock (the same entrance the Blackhawks players use).  The loading dock entrance is located on Damen Avenue just south of Madison Street. 
  • IT IS IMPERATIVE THAT ALL PARENTS AND OTHER FAMILY MEMBERS DO NOT  STAY IN THE LOADING DOCK AREA. Due to the heavy traffic of people in the loading dock area on a Blackhawks game-day, we must insist that parents do not stay in the loading dock area while their kids enter the United Center.  ONLY players and coaches will be allowed to wait at the loading dock for the rest of their team.
  • Players and coaches MUST have their Blackhawks game ticket in hand as they enter through the loading dock.  Individuals will not be admitted to the building without a ticket. Tickets will be torn and scanned in while teams are preparing for their game.

Family / friends / fans:

  • Please Use Parking: Lot K – Map attached – Regular parking for the United Center in Lot K is $25.00 this season.
  • Enter the United Center through Gate 6.  Gates will not open until 30 minutes before the Tomahawk’s  game.  Once you enter the parking lot and have paid, you may not exit and re-enter, and there will be no restroom available until the doors open 30 minutes prior to the Tomahawks game.
  • Tickets will be scanned and re-entry is not allowed.
  • There will be no one to scan tickets after the Tomahawks take the ice so if anyone is late, the building will be locked.
  • Tomahawk fans will be invited to sit in the designated 100-level (near the glass) seating area.  An “At Your Service” guest representative will assist in the seating process.
  • It is the policy of the United Center that video cameras are not permitted in the building. Photographic cameras are allowed.
  • POST-GAME:  Tomahawk fans must move out to the concourse area towards Section 114-115 where they will meet the Tomahawk players as they come up from the locker room.
  • The players will be escorted from the locker rooms up to the 100-level.  Parents will meet their kids outside Section 114 – 115. Tomahawks will carry their own equipment.  One person per Tomahawk, must go to Gate 7, have their ticket marked by the usher, exit, store their gear and return to Gate 7 with their specially marked ticket. You may only exit and re-enter once.  Show your ticket to the ushers upon entering the building.
  • YOU WILL NOT EXIT THE BUILDING.  YOU MUST STAY IN THE DESIGNATED AREA/ENTRANCE UNTIL THE GATES ARE OPEN FOR THE PUBLIC.  THE ONLY PEOPLE THAT NEED TO EXIT ARE THOSE TO STORE HOCKEY BAGS AND THEY MUST HAVE THEIR TICKET MARK BEFORE THEY EXIT THE BUILDING.

Parking Information

  • There are over 6,000 parking spaces in lots surrounding the United Center that can be accessed from Damen Avenue, Madison Street, Adams Street and Warren Boulevard
